Memorial Garden Brick Order Form

For a donation of $ 100 per brick you are invited to memorialize deceased loved ones in the walkway of our beautiful garden on the south side of the church. You may also make a $100 donation and have your own name engraved in support of your parish.

Carefully read these directions as you plan your memorial brick:

1. Each brick may have three lines of text
2. Each line may have up to 10 characters, including spaces
3. Print carefully on the lines below what you want on your brick
4. Include a check for $100 per brick with this order form in an envelope
5. You may mail the order form and check to St. Anne Parish Office, 6100 Arden, Warren, MI 48092 or

6. Please write "Brick" on the memo line of your check
7. Please submit this entire page as your order form. Additional forms are available in the Parish Office or this form may be duplicated
